Religious Education Registration Form, 2009 - 2010

Complete the form below to register your child/children for the Growing In Faith Together Religious Education program, grades 1-8, at Notre Dame parish. If your child is new to the program, or entering the first grade in the fall of 2009, a copy of his/her baptismal certificate is required. The tuition is $200 for one child, $350 for two children, $450 for three or more children. (You may deduct $15 if you register by June 1st.) Registration can be submitted on-line. The tuition payment can be mailed or dropped off to the Religious Education Office - or billed to credit card by calling the parish office for further information (630-654-3365). **Please note that currently registered families who do not enroll before August 1st will incur a late fee of $25.(The late fee does not apply for families who are new to the program.)

*A sacramental fee of $25 is required for students in Grade 2. (No child will be denied First Eucharist or First Reconciliation because of a family's inability to pay this fee.)

*If the tuition or sacramental fee presents a financial hardship for your family, please call the Religious Education Office for more information regarding tuition assistance.
