Religious Education Registration Form, 2008 - 2009

Complete the form below to register your child/children for the Growing In Faith Together Religious Education program, grades 1-8, at Notre Dame parish. If your child is new to the program, or entering the first grade in the fall of 2008, a copy of his/her baptismal certificate is required. The tuition is $200 for one child, $350 for two children, $450 for three or more children. (You may deduct $15 if you register before June 1st.) Registration can be submitted on-line. The tuition payment can be mailed or dropped off to the Religious Education Office - or billed to credit card by calling the parish office for further information (630-654-3365).
