Complete the form below to register your child/children for the Growing In Faith Together Religious Education program, grades 1-8, at Notre Dame parish.  If your child is new to the program, or entering the first grade in the fall of 2011, a copy of his/her baptismal certificate is required.  The tuition is $225 for one child, $375 for two children, $475 for three or more children. (There is an early bird discount if you register before June 1st.)  There is a sacrament activity fee of $30 for students in grade 2. The tuition payment can be mailed or dropped off to the Religious Education Office.  If you wish to pay by credit card, please call the parish office for further information (630-654-3365).
